Initial Consultation
Prior to your initial consultation, please have all pertinent medical records (imaging studies eg., MRI reports, CT scan reports, X-ray reports, referring doctor notes, etc.) sent to Pain Medicine Specialists of Arkansas. Call us at (501) 451-2500 for us to help you determine the best method for getting your medical records to us. We require that records be received prior to scheduling an initial consultation. Additional records may be requested if necessary.
For new patients, please read our Patient Forms page which provides you information about downloading the New Patient Packet forms. Also, new patients are required to present a photo ID and relevant insurance card at the time of the initial consultation.
Follow-Up Appointments
Check your patient portal to see if you have any new forms to fill out before your next appointment. Doing so will save you time at your next appointment.
